Supporting adults to live well in their community
Part of Wellbeing for Coventry, the Community Support Service offers personalised support for adults struggling with their mental health. Support workers can help individuals live well, manage their tenancy, support their own mental health, improve their social networks and live independently in the community.
Individuals can access support in-person within their home or a community setting. Sessions can also be delivered via telephone, video chat or themed groups depending on their preference. To ensure we are supporting individuals to meet their goals, we use the ‘Recovery Star’ model of support planning. This encourages individuals to plan their own support, identify areas they would benefit from and set achievable goals.
Support workers and volunteer befrienders will empower individuals to enjoy a good quality of life, covering areas including mental health, social networks, responsibilities, living skills, self-care, work, relationships, addictive behaviour and trust and hope.
Referral eligibility
Individuals with mental health needs who may be feeling isolated, struggling to integrate into the community, have had periods of hospitalisation and/or have ongoing mental health needs requiring support can be referred to this service by professionals.

The Pathfinder service provides the opportunity to anyone living in Coventry, over 18 years of age, who is experiencing an emotionally difficult time in their lives to identify the causes and explore ways to make positive changes. The service aims to assist in looking at all aspects of life and identifying what needs to change and what could be different. The service will signpost to specialist services when required while offering support in specific areas such as anxiety, low mood, negative thinking patterns, worrying, low confidence and many more.
